Work Schedule:Full Time Days Responsibilities: Support the development and management of care plans for participants in the Longmont Public Safety diversion program. Receive incoming referrals and triage by acuity and resource availability. Coordinate with public safety staff including patrol officers, paramedics and others. In coordination with other care providers, provide care to individuals suffering from substance use disorder and other behavioral health and/or chronic medical issues. Serve as a coach and role model to assist individuals in their recovery path. Coordinates with internal and external partners to access community resources and encourages clients to utilize these available resources and to be involved in their community. Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.Requirements:Credentials:* Licensed Clinical Social Worker* Licensed Professional Counselor* Licensed Marriage/Family TherapistOther information:Minimum Required Education: Master's degree in sociology, psychology, social work or similar field.Required Licensure/Certification: Must show proof of passage of Jurisprudence Exam and registration as an Unlicensed Psychotherapist or Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or Licensed Marriage and Family Counselor (LMFT) or Licensed Addiction Counselor (ACD) in the State of Colorado. Preferred: Valid driver's license.Minimum Experience: 2 years' experience in clinical social work or behavioral health care related to counseling or clinical case management and working with diagnoses of moderate to severe persistent mental illness. The pay range for this position is:$27.61 - $41.41 / hour.Pay is dependent on applicant's relevant experience.
