Job Description:

UCHealth is seeking a dedicated and compassionate Registered Nurse (RN) to specialize in Acute Care Transitions during the night shift, ensuring high-quality care and support during critical times of patient movement, either within our facilities or to home settings. This position demands a high level of adaptability and knowledge of acute care protocols, providing an essential link in our patient-centered care framework. By joining our team, you contribute to a mission-driven organization focused on improving lives and fostering healthy communities across Colorado. Assess patient conditions during transitions and collaborate with healthcare teams to execute seamless transfer plans. Monitor patient vital signs, manage medications, and implement doctor's orders during transitions. Educate patients and families about post-discharge care and necessary adjustments in treatment plans. Maintain accurate and up-to-date patient records. Participate in training and development activities to stay current with healthcare practices relevant to acute and transition care. Strong clinical skills in a hospital setting Excellent organization and communication skills Ability to work independently under minimal supervision Proficient in electronic medical record (EMR) systems Adept at handling patient needs with compassion and mindfulness Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Must possess a current RN license in the state of Colorado. BLS certification required. Preferred: ACLS and PALS certifications.
