Job Description:
AccentCare is seeking a dedicated Physical Therapist (PT) to
join our Home Health team, offering therapeutic treatments and
improvements in the quality of life for our clients at home. As a
Physical Therapist with AccentCare, you will have the unique

patients in their own environments. This role involves assessing
patient conditions, developing treatment plans, and implementing

lives. Conduct comprehensive patient assessments to establish
personalized care plans. Implement therapeutic interventions to
improve patient mobility and physical health. Track patient
progress and adjust treatments as needed. Educate patients and
families on home health care strategies and preventative measures.
Collaborate with healthcare professionals to provide holistic care.
Maintain patient records accurately and ensure confidentiality.
Expertise in physical therapy techniques and diagnostics Strong
interpersonal skills for effective communication with patients and
families Ability to work autonomously in a home setting
Problem-solving and critical thinking skills Capability to handle
