Contribute to the patients plan of care/service plan. Coordinate plans for patient discharge from services Coordinate care with other disciplines. Communicate effectively with other patients, families, field staff, office staff and physician offices. Job Requirements Active Registered Nurse (RN) License in Colorado Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the state(s) in which he/she practices.
